Change of Name Application Instructions
This is the changing of name form.
The applicant is to be the only one who signs with his or her old name. In the case of minors, parents/guardians are to sign, and there must be an Attested Legal Guardianship order, where possible.
All certificates should be self attested.
Applicant should be a native of this State and a declaration should be made.
There should be a logical cause of the change of the name.
Rs. 750/- (G.O. Ms.No.250, dated 11.10.2023).
In Person: e-Challan (no cash). Send counterfoil following payment.
Post: Payments by e-Challan only (not by DD/Cheque/Postal Orders). Send the form with counterfoil.
Forenoon: 10:00 a.m. to 1:00 p.m.
Afternoon: 2:00 p.m. to 3:00 p.m. (no holidays)
No reminders will be given.
Forms of this Department only will be accepted. No Xeroxes.
Check Door No. and PIN Code.
Give district of birth.
Punctuate initials and name correctly.
Life certificate should be attached by applicants 60+ years by a Government doctor.
The new name cannot be an alias name.
No birth certificate should be available, in that case, age certificate through a Government doctor.
Enclose a passport-size photograph with a self-attestation.
Go and collect Gazette within 5 days or it will be sent by post. In case of return, retrieve in person within 6 months.
